For Porting Your Existing Phone Numbers

In order to port (move) your existing telephone number(s) from your current Provider to 775.FYI, you must give us your written permission.  Please download, fill out and sign a Letter of Agency (LOA), below.

Please send the signed LOA along with a copy of your most recent phone bill to:  info@775.fyi
